Navigating the complexities of life with special needs or disabilities can often feel overwhelming for individuals and families. Since its establishment in 2022, Morgan’s MAC (Multi-Assistance Center) has been transforming the lives of individuals and families through its comprehensive and coordinated navigation services.

Designed as a pivotal hub for support, Morgan’s MAC employs a family-centered approach that fosters collaboration between families, MAC Members, and community partners who provide access to healthcare, social services, community, and more.

The heart of Morgan’s MAC is its unique MAC Care Model™, constructed to address Non-Medical Drivers of Health (NMDOH) and special care needs through a collaborative process of assessment, planning, facilitation, education, evaluation, and advocacy. Each individual, or MAC Member, is paired with a MAC Navigator, a single point of contact who is dedicated to guiding them through their personalized journey. The success of the Navigator’s role lies in creating a personalized care plan and serving as a trusted partner throughout their journey.

How Does It Work?

When a family comes to Morgan’s MAC, they are welcomed by a Navigator who performs a comprehensive NMDOH assessment using the MAC Care Model™. This assessment helps in identifying all areas of need and develops a tailored Morgan’s MAC Care Plan™. The Navigator ensures families receive the right referrals and support, tracking their progress every step of the way. With services both on-site and through a vast network of external partners, individuals and families can be assured they are receiving the coordinated care they need.

Navigating this journey can be challenging, but Morgan’s MAC is here to help every step of the way. Navigation services provided by Morgan’s MAC are completely free of charge, emphasizing their commitment to making a real difference without adding financial stress. The Navigator becomes a trusted partner, helping individuals and families feel seen and supported in their daily lives.

Morgan’s MAC is not just about solving problems—it’s about building a supportive community that understands and addresses the unique needs of each individual. Whether it’s through a friendly face, expert guidance, or comprehensive care, Morgan’s MAC is here to ensure that every individual with special needs and their families can lead their best lives.
