Transition planning is a person-centered and outcome-oriented process that promotes moving forward from school to post-school life and activities. This coordinated process is based on a student’s individual strengths, needs, preferences, and interests and the tools of self-determination to prepare a student for life as a contributing member of their community in work, learning, and independent life.
