A MESSAGE TO EX OFFICIO TRUSTEES of APH:

RE:    2015 APH Customer Satisfaction Survey for U.S. Department of Education

Link to survey: http://www.aph.org/products/gpra.html

Deadline: September 19, 2015

We need your input! APH is required to annually collect and report customer satisfaction data to the US Department of Education (DE) in compliance with the Government Performance and Results Act (GPRA). As part of this requirement, APH conducts a survey of professionals in education and rehabilitation as well as students and consumers and their family members to determine the effectiveness of our products and to gather this annual performance evaluation data. This 11-question, on-line survey offers you an opportunity to rate your satisfaction with our products and to express your thoughts and experiences with them. Your feedback will provide valuable input both for the Federal Quota Program and for the staff of APH.

As an Ex Officio Trustee of APH, your participation in this annual survey is critical. Some Trustees have asked if they should complete the survey if they do not use APH products directly. The answer is a loud “YES!”  The federal GPRA performance measures and the Dept of Ed require us to report separate response data for 1) the full pool of Ex Officio Trustees, 2) the Trustee Advisory Committees, 3) Teachers of the Visually Impaired, and 4) Other Consumers. …SO…YES! We need a response from you because of your vital role as an Ex Officio Trustee of APH. Please have your Federal Quota Account number(s) at hand when you submit your responses on line.

What if you do not personally use APH products frequently? It is acceptable for your responses to reflect the experience of professionals who teach students or clients registered through your account. It is also acceptable for you to respond only to the questions that you feel you are able to answer given the situation with your Federal Quota Account.

This survey is open to anyone who has used APH products in educational or rehabilitation settings. We encourage you to share this opportunity for input with your staff, other professionals, and all appropriate parties. Please help us spread the word.

We sincerely appreciate your sharing your time and expertise with us. You can respond to the survey by going to the link http://www.aph.org/products/gpra.html or visit the link on the products page of our website at www.aph.org/products/. The survey will be available through September 19, 2015.

If you have questions or require assistance, please email Mary Nelle McLennan at .

Thanks, as always, for your input and your continued support.
