Transition planning is a person-centered and outcome-oriented process that promotes moving forward from school to post-school life and activities. This coordinated process is based on a student’s individual strengths, needs, preferences, and interests and the tools of self-determination to prepare a student for life as a contributing member of their community in work, learning, and independent life. Access to services and supports outside the school realm is critical to helping students and families address the challenges of planning.
